My child/children, the registered participants and I (Parent and Guardian) agree to adhere to the rules, regulations and conditions prescribed by the "Showcase Hockey Development Camp" organizers and staff. I agree to waive responsibility of "Showcase Hockey Development Camp", including Showcase Hockey Development Camp staff and volunteers, from any occurrence inclusive or accidents, injury, damage to personal property or theft that may occur during the camp. By signing below, I acknowledge that I have read and agree to the terms of the waiver.
